Duties Description

The Registered Nurse 2 Community oversees the medical care for individuals living in Family Care (FC) homes. They are required to visit each home as frequently as health care needs warrant but in no case less than once every 90 days with the individual present. The candidate will communicate routinely with FC Provider to provide support as needed. They will participate in the development of recommendations related to home modifications and adaptive equipment as appropriate. They will assess FC Provider skills and abilities relative to addressing the needs of individuals in their home. Ensures Medical Record and Ready to Go Binder in FC home contains all required documents, is orderly and current for each individual in the home. Candidate will coordinate appointments for individuals, ensure follow up and documentation from appointments is obtained. Tasked to accompany individuals and FC Providers to specialty appointments and assist with transportation when necessary and as needed. During a hospitalization the RN2 Community is required to follow up with hospital personnel regarding the individual and provide updates on a consistent basis. They are required to visit the hospital frequently when the individual has been admitted.

The candidate will complete and maintain all Nursing documentation required per agency policy and regulations, provide oversight and training in medication administration, infection control, CPR, first aid and any other health related topics. They will function as part of the FC Team, as well as communicate and interact with medical professionals and Care Coordinators in the community. The position requires some travel and there may at times be a small caseload in the Newark area as well. The candidate will attend QLR's, Life Plan and ISP reviews, as well as actively participate with respite situations. They will be responsible for assisting FC Providers in covering the cost of medically related purchases using a State VISA card, and therefore, responsible for maintaining records of purchases. Reviews FC Providers Medicaid reimbursable transportation for accuracy. Provide Nursing coverage for other FC Nurses within the department. Obtain Infection Control training for Nurses per licensing requirement.

Minimum Qualifications

Applicants must possess a license and current registration as a registered professional nurse in New York State; and two years of post-licensure clinical nursing experience. One year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.

Substitution:

A bachelor's degree in nursing may substitute for one year of general post-licensure clinical nursing experience; and a master's degree in nursing may substitute for two years of general post-licensure clinical nursing experience.

This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.

At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.

  • Posted Hiring Salary includes the sum of the statutory hiring rate for PS&T Grade 20 ($74,193), Increased Hiring Salary ($9,980) and Geographic Pay Differential ($20,000). Geographic Pay Differential is added to the job rate ($94,121).
  • If you are currently a New York State employee serving in a permanent competitive qualifying title and eligible for a transfer via Section 70.1 of the Civil Service Law, you may be appointed to this title as a permanent competitive employee.
  • The assignment of PS&T employees will be made in accordance with any agreed upon Memorandum of Understanding between PEF and the local DDSO regarding the posting of job vacancies.
